HelpAge: Community-based response in prevention and management of non-communicable diseases in old age, Phase II

Description

The project aims to bridge the gap in government service delivery and unmet needs of older people through the inclusion of services provided by civil society networks. The model focuses on prevention and the management of chronic conditions of older people with older volunteers acting as support agents for family doctors. The main objectives of the project are to improve awareness among policy makers and service providers at all levels about vulnerability of older people accessing healthcare services in rural areas, as well as to strengthen capacity of civil society networks and state authorities to provide effective community-based health-care support to vulnerable older people and also to increase knowledge of older people to manage their health status and access health and social services by vulnerable older people.
